Elderly & Disabled Waiver Program (EDWP)

CCSP (Community Care Services Program): The Community Care Services Program (CCSP) helps frail, elderly individuals stay in their homes or communities instead of moving to a nursing facility. While CCSP is a Medicaid waiver, it is not the same as Medicaid. To receive Medicaid coverage through CCSP, clients must actively receive at least one CCSP service. If services stop, the Medicaid benefit ends as well. Some clients may be required to pay a cost share (co-pay) for their services.

SOURCE (Service Options Using Resources in a Community Environment): Service Options Using Resources in a Community Environment (SOURCE) is a State Plan enhanced primary care case management program that serves frail elderly and disabled beneficiaries to improve the health outcomes of persons with chronic health conditions, by linking primary medical care with home and community-based services.

Services offered by Citadel Community Health Services under CCSP & SOURCE are:

  • Personal Support Services – Offers personal care, medically related transportation, housekeeping and other services as ordered by the Care Management Team.
  • Extended Personal Support Services – Provides in-home respite and assistance with personal care, meal preparation, light housekeeping and home safety.
  • Skilled Nursing – Examples of such services include wound care and medication management.

The NOW waiver program offers services and support to individuals to enable them to remain living in their own or family home and participate or live independently in the community.

The COMP waiver program serves individuals with more intensive needs. These individuals require comprehensive and intensive services and need out-of-home residential support and supervision or intensive levels of in-home services to remain in the community.

We provide the following services under NOW & COMP;

  • Community Living Support (CLS) – In-home and community-based support to develop daily living skills and promote independence.
  • Community Access Individual (CAI) – One-on-one services to build social skills, access the community, and achieve personal goals.
  • Community Access Group (CAG) – Small-group outings and activities that encourage community inclusion and social engagement.
  • RN & LPN Nursing – Skilled nursing for individuals with medical needs, provided in the home by licensed professionals.
